With no exception, all individuals purchasing Electrotherapy Instruments must fax a copy of their Doctor's Prescription before ordering. Orders will not be shipped without the proper documentation.
Thirty (30) Day Return Policy:
Damaged or defective products:
All of your purchases at Chattanooga Medical Supply are covered for product defects for a full 30 days after receipt by you. In the unlikely event that you should receive defective merchandise, please contact our Customer Service Department immediately to determine the best method of product replacement or repair.
Beyond 30 days, you should contact the manufacturer for repair or replacement. This manufacturer contact information will usually be found on the box or in your warranty/instruction manual.If help is needed please don't hesitat to call us.
RETURN AUTHORIZATION
A Return Authorization Number (RA#) must be obtained from Chattanooga Medical Supply,Inc. before any items are returned in order for items to be accepted. All returns are subject to a 15% restocking fee. Shipping fees are non refundable.
RETURN EXCEPTIONS
Unfortunately, there are some products for which we are unable to accept returns or refunds, e.g., special order or production items, rush order items, etc. Please inquire of any items in question.
